STATEMENT OF WORK DENTAL REMOVABLE PROSTHDONTICS LABORATORY SERVICES Removable Dental Prosthetics Services OBJECTIVES: An overview of this SOW will include goals and objectives. The Hampton VAMC North Battlefield HCC has a need to continuously utilize dental removable prosthodontics laboratory services for Veterans requiring special dental lab work. General Requirements The contractor will provide laboratory support for removable prosthodontic laboratory work for Hampton VAMC North Battlefield HCC in Chesapeake, VA. The contractor shall provide all materials and supplies necessary to complete each item. Acceptable supplies for removable prosthodontics include type III gold, noble metals, Ivocap Injectable Acrylic Resin or equivalent and Trubyte Portrait IPN denture teeth. Specific Requirements The contractor will use procedures, techniques and current technology that are the standards in the industry. The contractor shall provide items that meet the esthetic and functional requirements of the patient. The contractor shall be familiar with mould guides by Trubyte Bioblend and with shade guides by Trubyte, Bioform, and the VITA 3D Toothguide. High quality materials shall be utilized from ISO 9001/CE registered companies. Use high quality raw materials purchase from ISO 9001/CE registered companies. Laboratory shall perform internal technical audits to ensure consistent, high quality, and timely restorations. This includes the use of Dental Wave 7000 Microwave burnout technology, CAD CAM technology & advanced laser welding. The contractor shall provide the following dental laboratory services to the Hampton VAMC North Battlefield HCC in Chesapeake, VA: Complete Dentures (including bite rims, set-up, process, and finish) Cast Partial Dentures (including frameworks, bite rims, set-up, process, and finish) Acrylic Partial Dentures (including articulation) Hybrid Cast/Flexible Partial Dentures Complete and Partial Denture Repair Complete and Partial Denture Relines and Rebases Diagnostic Wax-up and Design Services for Removable Prosthodontics Thermoplastic Occlusal Guards Flexible (Flexite/Valplast Type) Partial Dentures Swing-Lock Type Partial Dentures Removable Overdentures Surgical Stents ID Dentures and Partial Dentures Implant Bar Overdenture Precision Cast Retentive Bars Precision and Semi-Precision Attachments Fluoride Application Trays The following materials shall be used in the fabrication of dental prostheses: Ivocap Injectable Acrylic Resin Coelor Coloration or Equivalent Occlusal Scheme: Monoplane, linear Anatomic, full-balanced Lingualized, linear 33*/monoplane 30* rational Functional/rational Tooth Material: Trubyte Protrait IPN The contractor shall identify the specific type of metal used in each prosthesis at the time of delivery. The contractor shall provide a thorough infection control process for all incoming and outgoing cases. All completed services must be disinfected before the product is returned to Hampton VAMC North Battlefield. The contractor shall provide local repair and reline service with same day service turnaround for all denture repairs and relines (excluding framework repairs) The contractor shall provide, at no additional cost to the VA, professional case consultation with responsive and knowledgeable customer service. The contractor will provide personalized pick-up and delivery services to ensure that casework arrives on time and safely at no additional cost to the Hampton VAMC North Battlefield. The attached Quality Assessment Surveillance Plan of Dental Laboratory Procedures will be used to measure/assess the quality of the product produced (See Section D). The laboratory must provide same day pickup and delivery service between the VA and the dental laboratory with a laboratory provided courier. No case work will be mailed as dental wax distorts and flows to an unacceptable level in hot weather and contracts and cracks in cold weather. Impressions, teeth, prescriptions, finished products, etc. will be picked up by laboratory provided courier from the Hampton VAMC North Battlefield Dental Service in Chesapeake, Virginia transported to laboratory and then delivered back to Hampton VAMC North Battlefield Dental Service in Chesapeake, Virginia daily, Monday through Friday, excluding Federal Holidays. Pick-up and delivery shall be between the hours of 8:00 a.m. to 4:00 p.m. The laboratory must perform all services in-house except for removable partial denture metal frameworks; otherwise, no trans-shipping is allowed. All work, to include metal frameworks, must be performed within the borders of the continental United States of America. Service Turn-around Time Completed casework shall be returned to Hampton VAMC North Battlefield Dental Service in Chesapeake, Virginia. The Hampton VAMC North Battlefield Dental Service desires that work be returned when completed via daily deliveries, Monday through Friday, excluding Federal Holidays. Pick-ups and deliveries must be accomplished between the hours of 8:00 a.m. - 4:00p.m. The contractor shall adhere to the following turn-around requirements: All complete and partial denture steps shall be completed no later than five (5) working days of pickup except for removable partial denture frameworks, which may take up to fifteen (15) working days. All work must meet the required turn-around times. Fifteen (15) Working Days: Removable Partial Denture, framework Five (5) Working Days: Complete and Removable Partial Denture, process acrylic resin Obturator, process acrylic resin Tooth Arrangements, Complete and Removable Partial Dentures Occlusal Night-guard, processed acrylic resin Diagnostic wax-up, five teeth or more Three (3) Working Days: Diagnostic wax-up, four teeth or less Tooth Rearrangements, Complete and Removable Partial Dentures Two (2) Working Days: Complete and Removable Partial Denture, record bases and occlusion rims Complete and Removable Partial Denture, record bases and occlusion rims, stabilized/processed Transitional Removable Partial Denture, processed Custom impression trays Custom impression trays, altered cast impressions Poured impressions Articulation of casts Cast duplications Fluoride application trays Occlusal night-guard, vac-u-form Complete denture and removable partial denture relines Complete denture and removable partial denture rebases Same Day Service: Complete and Removable Partial Denture acrylic resin repairs Complete and Removable Partial Denture, tooth additions/repairs Removable Partial Denture, clasp repair/replacement and wrought wire clasp retained by acrylic resin Hampton VAMC North Battlefield Dental Service VA Requirements Dentists will provide legible dental service prescriptions. Hampton VAMC North Battlefield dentists will provide accurate impressions and bite registrations. If a prescription is neither legible nor clearly understood, the contractor shall contact VA s Contracting Officer s Representative (COR) to obtain a clear and legible prescription. Impressions, teeth, prescriptions, finished products, etc. will be picked up by laboratory provided courier by the Hampton VAMC North Battlefield Dental Service, Richmond VA and securely transported to the contractor's laboratory in a temperature-controlled environment. The contractor is to use secure transport methods to ensure security, safety, temperature restrictions and fragility of laboratory items, as well as on time delivery. Completed work shall be immediately returned to the Hampton VAMC North Battlefield Dental Service. The Hampton VAMC North Battlefield Dental Service desires that work be returned when completed with daily deliveries, Monday through Friday, excluding Federal Holidays. Pick-ups and deliveries must be accomplished between the hours of 8:00 a.m.-4:00 p.m. Corrective Actions Contractor: The contractor shall, at no additional expense to VA, remake all dental items that are not correct or flawed due to contractor errors. For example, if a framework does not fit the master cast it will be considered flawed and unacceptable. The framework will be remade at no additional expense to the government. If, in the case of acrylic resin denture work, the resin cracks or the tooth de-bonds within twelve months of fabrication, it shall be considered flawed and remade at no additional expense to VA. If, in the case of removable work, the teeth are not set per the dentist s instructions, or if the occlusion is incorrect on the articulator, it shall be considered flawed and remade at no additional expense to VA. Hampton VAMC North Battlefield, Chesapeake: For removable cases, if the occlusion is accurate on the articulator, but not accurate in the mouth, it will be considered a flawed action of the government, and the remake will be at VA s expense. PLACE OF PERFORMANCE Removable Dental prosthetics services will be performed by the vendor at their respective inspected and approved facility. CONTRACTOR EXPERIENCE REQUIREMENTS: Qualifications of key personnel: Work must be performed by Certified Dental Laboratory Technicians. The Contractor Officer or designer reserves the right to thoroughly inspect and investigate the contractor s and/or subcontractors facilities and other qualifications. The contractor facility shall have all licenses, permits, and certification required by law. Current copies of these must be provided to the Contracting Officer upon request. The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee s performance is unsatisfactory. All contract employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. Include: Technical Proficiency The Certified Dental Laboratory Technician(s) shall be technically proficient in the skills necessary to fulfill the government s requirements, including the ability to speak, understand, read and write English fluently. Contractor shall provide documents upon request of the CO/COR to verify current and ongoing competence, skills, and training related to the provision of care, treatment and/or services performed. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history. QUALITY CONTROL The Contractor shall develop a Quality Assurance Plan (QAP) which will provide the Contractor s strategy for monitoring performance internally, providing performance information to the Government, and reporting in compliance with the PWS. Within the QAP, the Contractor shall propose and maintain an established reporting system to promptly notify VA of any problems, issues, risks, or concerns with workflow performance, programmatic improvement, security, and schedule adherence. Quality control is the exclusive responsibility of the Contractor. The final plan shall be submitted to the CO and COR within 30 days of contract award. QUALITY ASSURANCE SURVEILLANCE PLAN The Government intends to utilize a Quality Assurance Surveillance Plan (QASP) to monitor the quality of the contractor s performance. The oversight provided for in order and in the QASP will help to ensure that service levels reach and maintain the required levels throughout the contract term. Further, the QASP provides the COR with a proactive way to avoid unacceptable or deficient performance and provides verifiable input for the required Past Performance Information Assessments. The QASP will be finalized immediately following award and a copy provided to the Contractor afterward. The QASP is a living document and may be updated by the Government as necessary. KEY PERSONNEL Key personnel will be identified in the proposal and shall be considered key personnel essential for the successful completion of the work done under the contract. The contractor agrees that such personnel shall not be removed, diverted or replaced from the work without prior written approval of the Contracting Officer. The contractor shall submit written notice of proposed personnel changes to the Contracting Officer for approval at least thirty (30) business days in advance. WORK HOURS: The services covered by this contract shall be furnished by the contractor as defined herein. The contractor shall not be required, except in case of emergency, to furnish such services on a Federal Holiday or during off duty hours as described below. The following terms have the following meanings: Normal working hours: Monday through Friday, 8:00 a.m. 4:30 p.m., excluding federal holidays. Federal Holidays: The 10 holidays observed by the Federal Government are: New Years Day Martin Luther King s Birthday President s Day Memorial Day Juneteenth Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Any other day specifically declared by the President of the United States to be a federal holiday. When one of the holidays falls on Sunday, the following Monday shall be observed as a Federal Holiday. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday. Also included would be any other day specifically declared by the President of the United States to be a National Holiday.
