Background
The Department of Veterans Affairs (VA) is issuing a competitive Request for Quotation (RFQ) for a follow-on contract to provide community nursing home services for eligible veterans in the Avoyelles Parish, Louisiana area.

This contract aims to replace the previous contract 36C25620D0052 with Bayou Chateau CNH and is part of the VA's mission to ensure quality care for veterans through its Community Nursing Home (CNH) program. The contract will be funded under authority 38 U.S.C. 1720 and will utilize Medicaid rates for reimbursement.

Work Details
The contractor will be responsible for providing comprehensive community nursing home services, which include:
1. Furnishing all personnel necessary to perform nursing home services at per diem rates established by Medicaid plus a fair market amount.
2. Ensuring that all nursing home facilities have current Center for Medicare and Medicaid Services (CMS) certification and state nursing home licenses.
3. Providing care that meets the medical, nursing, and psychosocial needs of VA beneficiaries, including room, meals, nursing care, and other necessary supplies without additional charges.
4. Accepting referrals from the VA within a timely manner (ideally within 24 hours).
5. Conducting regular quality oversight visits by VA representatives to ensure continuity of care.
6. Reporting any significant changes in patient status or facility conditions immediately to the VA.
7. Complying with all federal, state, and local labor laws applicable to this procurement.

Period of Performance
The period of performance for this contract is anticipated to be from July 1, 2025, through June 30, 2026.

Place of Performance
The services will be performed in Avoyelles Parish, Louisiana.
