Background
The Department of Veterans Affairs has established the Health Care for Homeless Veterans (HCHV) program in accordance with 38 USC 2031 and 38 CFR Part 63. This program aims to provide care, treatment, and rehabilitative services to homeless veterans through the Southern Arizona VA Healthcare System (SAVAHCS).

The central goal of the HCHV program is to reduce homelessness among veterans by conducting outreach to vulnerable individuals who are not currently receiving VA services and engaging them in treatment and rehabilitation.

Work Details
The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary to perform Health Care for Homeless Veterans services. Specific tasks include:

- Obtaining a Release of Information from veterans;
- Ensuring documentation meets CARF standards;
- Providing clinical assessments within seven days of admission;
- Offering case management services;
- Conducting brief therapeutic interventions such as Cognitive Behavioral Therapy (CBT);
- Assisting veterans with housing applications;
- Providing transportation assistance; and
- Ensuring a safe environment for all veterans.

The contractor must also provide routine updates to treatment plans and document progress notes detailing each veteran's goals and participation in treatment.

Period of Performance
The contract will be performed over an initial award period from January 1, 2025 through December 31, 2025, followed by two additional ordering periods from January 1, 2026 through December 31, 2026 and from January 1, 2027 through December 31, 2027.

Place of Performance
The work will be performed at the Contractor facility within the City of Tucson or South Tucson city limits.
