Background
The Department of Veterans Affairs (VA) is issuing this solicitation for Medical Respite Care services under the Health Care for Homeless Veterans (HCHV) Program. The goal of this contract is to provide temporary housing and supportive services in a community-based setting for Veterans experiencing homelessness, particularly those with special needs. This program aims to facilitate access to a broad range of medical, mental health, and rehabilitative services, ensuring a safe and secure environment that promotes rehabilitation and transition to permanent housing.

Work Details
The contractor shall provide comprehensive Medical Respite Care for homeless Veterans, including:
- Staffing 24/7 with at least one administrative staff member available at all times for emergencies.
- Accommodating a minimum of three homeless Veterans requiring medical and mental health respite care.
- Providing all necessary personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items required to perform the services as defined in the Performance Work Statement (PWS).
- Conducting clinical assessments to determine each Veteran's strengths, needs, abilities, and preferences.
- Initiating case management to plan and coordinate care for homeless Veterans.
- Offering therapeutic and rehabilitative services tailored to address the underlying factors contributing to homelessness without providing detoxification or acute treatment.
- Developing individual treatment plans with specific goals and measurable objectives aimed at improving safety, mental health status, employability, quality of life, self-esteem, independence, and preventing homelessness.
- Ensuring regular updates on treatment plans and documenting progress notes that include participation in treatment and changes to service plans.

Period of Performance
The contract will have a base period of performance from September 1, 2024, to August 31, 2025, with four optional renewal periods of one year each.

Place of Performance
Services will be performed at the Southern Arizona VA Healthcare System (SAVAHCS), specifically within Tucson and South Tucson city limits.
