SOURCES SOUGHT NOTICE Community Nursing Home Services Indefinite Delivery Contract (IDC) Southeast Louisiana Veterans Health Care Systems (SLVHCS) New Orleans, LA 70119 DESCRIPTION: This is a sources sought to determine the availability of potential sources having the skills and capabilities necessary to provide the appropriate level of Community Nursing Home (CNH) care to the beneficiaries the Department of Veterans Affairs, Southeast Louisiana Veterans Health Care Systems (SLVHCS), New Orleans, LA 70119, has a requirement for one (1) Nursing Homes in Covington, Louisiana, catchment of St. Tammany Parish. The estimated period of performance will be from the Award Date for a 5-year ordering period to renew and shall be exercised at the sole discretion of the Government. The number of Veterans requiring Community Nursing Home services (bed days) over the next five years will fluctuate and cannot be adequately determined at this time. Therefore, the award of a Firm-Fixed Price Indefinite Delivery Contract (IDC) is anticipated. Please be aware all Federal, State and local labor laws will be applicable to this procurement including, federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act. If you have specific questions regarding labor laws, you should contact your local Federal Department of Labor representative. REQUIRED: Interested agencies will be Medicaid as applicable to the location as a Certified Nursing Home Facility, licensed as a Nursing Home by the State of Louisiana in which its provider number is based, accreditation by a recognized accrediting body i.e. Joint Commission (JC) or Community Health Accreditation Program (CHAP). All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought notice. Questions should be submitted by email to sharrion.young-mosley@va.gov. Provide only the requested information below. The purpose of this notice is to gain knowledge, in this order, potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 623110 Nursing Care Facilities (size standard $27.5 M). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this source sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this source sought announcement. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company can provide the requested services at time of award. Include your company s experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small as defined by the NAICS (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service- Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Page 3 of 3 Include the DUNS number of your firm. (h) In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in VetBiz database if applicable (VetBiz registration is required for Service-Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Contractors may obtain SAM information at https://www.sambeta.gov and Vet Biz information at http://www.vip.vetbiz.gov. To be eligible for award, contractors must be registered in the System for Award Management database. Registration is free and potential offerors are encouraged to visit the SAM website as no IDCs can be awarded to any offeror not registered in SAM. Responses must be emailed to sharrion.young-mosley@va.gov no later than 4:00PM (CST), Monday, July 26, 2021. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO- ECONOMIC STATUS information as explained above. Please place "Attention: CNH Covington, LA in the subject line of your email. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced later, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
