Background
The Department of Veterans Affairs, Network Contracting Office 16 (NCO 16) is seeking a contractor to provide Community Nursing Home Services in Lonoke County, Arkansas. This contract aims to support the Central Arkansas Healthcare System (CAVHS) by ensuring that veterans receive necessary nursing home care as part of the Veterans Health Administration's continuum of care.

Work Details
The contractor is responsible for providing Community Nursing Home (CNH) Services for the Department of Veterans Affairs (VA) Central Arkansas Healthcare System (CAVHS) within Lonoke County. The services include:
1. Ensuring that care meets the health needs and promotes the maximum well-being of VA patients.
2. Providing total medical, nursing, and psychosocial needs of VA beneficiaries.
3. Maintaining current Center for Medicare and Medicaid Services (CMS) certification and a State nursing home license.
4. Ensuring bed capacity to accept VA referrals in a timely manner, ideally within 24 hours of request.
5. Providing at least one provider visit per month and access to laboratory, x-ray, and other special services as needed.
6. Offering room, meals, nursing care, and other services or supplies at no extra charge according to the VA-authorized level of care.
7. Conducting quality oversight visits by authorized VA staff to ensure continuity of care.

Period of Performance
The anticipated period of performance includes two ordering periods: 1st Ordering Period from March 1, 2025 through February 29, 2026; and 2nd Ordering Period from March 1, 2026 through February 28, 2027.
