Description

This is a request for proposals for establishing and operating a CBOC in El Centro, California. Please see attached RFP and all associated attachments.

Background
The Department of Veterans Affairs San Diego Healthcare System is seeking proposals for the establishment and operation of a Community Based Outpatient Clinic (CBOC) in El Centro, California. This initiative aims to provide primary care services to veterans, aligning with the agency's mission to enhance healthcare access and quality for veterans.

Work Details
The contractor will be responsible for providing comprehensive primary care services for veterans at the CBOC in El Centro. This includes:
1. Delivering accessible, coordinated, comprehensive, patient-centered care in a team-based environment.
2. Staffing requirements include a Patient Aligned Care Team (PACT) consisting of Primary Care Providers (PCPs), Registered Nurse Care Managers (RNCMs), Clinical Associates, and Administrative Associates.
3. Ensuring that each PCP has a caseload ratio that meets VA standards (1200 active patients per full-time physician and 900 active patients per full-time midlevel provider).
4. Implementing contingency plans for continuity of care during staffing shortages or emergencies.
5. Compliance with VA security and privacy regulations, including the handling of sensitive information and adherence to HIPAA requirements.
