See attached Request for Proposal (RFP) 36C24423R0132 for Community Based Outpatient Clinic (CBOC) services, and RFP Attachments D.1-D.19. Lebanon VA Medical Center, also known as the parent facility, requires CBOC services providing Primary Care in a private hospital, office or clinic environment to Veterans, primarily residing in Schuylkill County, with the physical location being in the city limits of Pottsville, Pennsylvania in accordance with the terms and conditions included in the RFP. The subsequent contract will be performed for the Department of Veterans Affairs, Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042-7529. As a result of the solicitation, the Government intends to award one (1) indefinite-delivery, indefinite-quantity, (IDIQ) Contract, made to one offeror who can successfully place a competitive proposal to operate a CBOC in the desired location. This solicitation will be set aside 100 percent for Service Disabled Veteran Owned Small Business competition. Potential awardees must be certified Services Disabled Veteran Owned Small Businesses, meaning a Service Disabled Veteran Owned Small Business that has been certified in the SBA Veteran Small Business Certification Program and listed in the SBA certification database (see 13 CFR 128.102). Additionally, contractors must be registered with Vets 4212 at VETS-4212 Federal Contractor Reporting | U.S. Department of Labor (dol.gov). All Contractors must be registered in System for Award Management at www.sam.gov to be eligible for award and complete their online Representations and Certifications at www.sam.gov. The applicable NAICS code is 621498 with a size standard of $22.5 million in average annual receipts. Award will be made to the offeror whose proposal is responsive and conforming to the RFP and are also determined to be most advantageous to the Government. The selection process will be conducted in accordance with FAR Part 15 source selection procedures. Proposals will be evaluated on Technical Capability, Past Performance, and Price Evaluation of the offeror s response to the solicitation. Background
The Department of Veterans Affairs, Lebanon VA Medical Center, is seeking Community Based Outpatient Clinic (CBOC) services to provide Primary Care in a private hospital, office, or clinic environment to Veterans residing in Schuylkill County, primarily in the city limits of Pottsville, Pennsylvania.

Work Details
The purpose of this solicitation is to procure CBOC services for VA patients in the vicinity of Schuylkill County, PA. The subsequent contract anticipates awarding an Indefinite Delivery Indefinite Quantity (IDIQ) contract with a 5-year base performance period and five option years to be exercised at the VA's discretion. The Contractor shall be responsible for the correct title classification of workers and compliance with all applicable wage and hour laws.
The solicitation will be set aside 100 percent for Service Disabled Veteran Owned Small Business competition. Proposals will be evaluated on Technical Capability, Past Performance, and Price Evaluation.

Period of Performance
The contract will have a 5-year base performance period and five option years to be exercised at the VA's discretion.

Place of Performance
The CBOC services are required to be performed within the city limits of Pottsville, Pennsylvania 17901.
