Solicitation Number: 36C79121R0008 Remote Patient Monitoring Home Telehealth (RPM-HT) FY22

Synopsis: The Department of Veterans Affairs (VA), Commodities & Services Acquisition Service (CSAS), 555 Corporate Circle, Golden, CO 80401, has a requirement to provide Remote Patient Monitoring Home Telehealth (RPM-HT) services/supplies to Veterans. This requirement includes necessary platforms and peripherals to ensure healthcare needs are met while ensuring generated data remains secure. The applicable North American Industry Classification System (NAICS) is 621999 and the size standard is $16.5 million. We anticipate issuing a Request for Proposals (RFP) approximately on or about September 07, 2021 on Contract Opportunities (www.sam.gov), but all dates in this notice are subject to change. Any amendments applicable to the solicitation will be posted on Contract Opportunities. To take advantage of business opportunities with the Department of Veterans Affairs, you must register your firm with the System for Award Management (SAM) at www.sam.gov AND in the VetBiz database if applicable, (VetBiz registration is required for Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Prospective offerors are encouraged to register their interest in the solicitation on the Contract Opportunities website in order to receive notifications of any solicitation and/or amendments postings. Registration for SAM is free and potential contractors are encouraged to visit the SAM website as no contract can be awarded to any offeror not ACTIVE in SAM. The VA intends to award up to four (4) firm fixed-price, Indefinite-Delivery, Indefinite-Quantity (IDIQ) contract(s). Resulting awards will consist of a two-year (2) base award with six (6) option years. This requirement will be solicited as a Full and Open Competition with Best Value Trade Off determination that will consider Factors under Technical, Past Performance and Price. Final evaluation factors will be defined in the solicitation under Section E. Background
The Department of Veterans Affairs (VA) Commodities and Services Acquisition Service (CSAS) and the Denver Logistics Center (DLC) manage a holistic supply and service chain for the VA National Audiology, Prosthetics, and Telehealth Programs and support the Veterans Health Administration Office of Connected Care.
Under the VA Mission Act, VA requires Remote Patient Monitoring Home Telehealth (RPM-HT) services and supplies to enable eligible Veterans to self-manage health conditions through digital health technologies.
The program aims to improve the quality and efficiency of care, increase patient satisfaction, and ensure secure integration of generated data with VA systems.

Work Details
The contractor shall supply and support commercial off-the-shelf RPM-HT platforms and peripherals that collect and transmit vital signs including blood glucose, blood pressure, pulse, temperature, weight, pain via Likert scale, pulse oximetry, and daily Disease Management Protocol responses.
Tasks include hosting the IT infrastructure in VA on-premises data centers (Hines, IL and Austin, TX) and VA Enterprise Cloud.
The contractor will provide high-level architecture and data flow diagrams and deploy a Care Coordinator Web Viewer for patient enrollment, care category assignment, alert threshold configuration, risk stratification, session scheduling, and historical data review.
Integration via HL7 and API interfaces to VA’s electronic health record is required, along with establishing secure networks and applying FISMA, HIPAA, HITECH, Privacy Act, FDA and Section 508 compliance.
The contractor must develop and execute a Configuration Management Plan and a Quality Control Plan, perform customer service and training, manage supply chain functions including ordering, delivery, packaging, and electronic invoicing, and generate required monthly, quarterly, and ad hoc reports.
The contractor will remediate security vulnerabilities within mandated timeframes and participate in quarterly alternate site switchovers per the Performance Requirement Summary.

Period of Performance
Two-year base period with six one-year option periods.

Place of Performance
Services to Veterans will be performed throughout the United States, its territories and possessions (American Samoa, Guam, Northern Mariana Islands, U.S. Virgin Islands), the District of Columbia, and Puerto Rico.
RPM-HT systems will be hosted at VA data centers in Hines, Illinois and Austin, Texas, and within the VA Enterprise Cloud.

Bidder Requirements
Offerors must submit a completed, signed and dated SF-1449 with the Pricing Schedule and include their DUNS.
Offerors must propose and commit the following key personnel with a minimum of five years of relevant experience and required professional registrations or certifications: Project Manager, System Administrator, Database Administrator, Information Security Officer, Software Developer, Clinical Program Director, and a Change Management/Release Management certified employee.
Offerors must provide three-page resumes and letters of commitment for each key personnel.
Offerors must comply with FISMA, HIPAA/HITECH, the Privacy Act and VA security directives.
Offerors must furnish an organizational Configuration Management Plan, a Quality Control Plan, and a data breach plan with breach history.
Offerors must submit up to ten past performance references with completed questionnaires and provide required platforms and peripherals for the demonstration phase.
