Page 5 of 5 Page 1 of SYNOPSIS OF AWARD May 1, 2023 The Department of Veterans Affairs, Commodities Services and Acquisition Services (CSAS), in conjunction with Office of Connected Care, and the Denver Logistic Center located at 555 Corporate Circle, Golden, CO 80401, has awarded four (4) contracts against solicitation number 36C79121R0008. These contracts are for Home Telehealth Technologies to be provided in the entire United States. Interested parties were solicited through Contract Opportunities (www.sam.gov) as a full and open competition.

The solicitation was posted on September 9, 2021 and proposals, following RFP amendments, were due on October 25, 2021. The requirement is for Remote Patient Monitoring - Home Telehealth (RPM-HT) which is a service with ancillary supplies that currently supports the care of over 72,000 patients with chronic care, acute care, health promotion/disease prevention, and non-institutional care (NIC) needs, and is required by the Office of Connected Care. RPM-HT supplies and their associated services are Prosthetic Appliances under this definition because they are required to manage and support care coordination and disease management.

RPM-HT technologies gather and provide critical Veteran health information that is utilized by their health care team, in collaboration with the Veteran, to reduce impairments, promote healthy outcomes and control disease management.

The RPM-HT multiple award contracts have historically been awarded using a Firm Fixed Price (FFP) contract type, IDIQ, under North American Industry Classification System (NAICS) Code: 621999 All Other Miscellaneous Ambulatory Health Care Services; Product Service Code (PSC): 6515 Medical and Surgical Instruments, Equipment, and Supplies.

Pursuant to FAR 16.504(a), the total maximum quantity of supplies and services under the basic contract for all awardees combined shall not exceed $1.032 billion, including the base and all option periods. Each of the resulting contracts will include a base period of two (2) years and six (6) option periods upon approval of Authority to Operate is granted.
