Background
The Department of Veterans Affairs (VA) is seeking to provide Community Based Outpatient Clinic (CBOC) Services in Dyersburg, Tennessee, primarily for veterans residing in Dyer County.
This initiative is part of the VA's mission to enhance healthcare delivery through increased access and coordination of care.
The contract aims to establish a facility that will deliver both primary and mental health care services, thereby supporting the VA's Patient-Centered Medical Home (PCMH) model known as PACT, which emphasizes comprehensive, patient-centered care.

Work Details
The contractor shall provide comprehensive CBOC services including:
- Primary Health Care Services: Monthly capitated rate for primary care visits.
- Mental Health Care Services: Monthly capitated rate for mental health visits.
- Injectable Medications: Administered at the CBOC which are not listed in the contract.
- The contractor must ensure compliance with VHA performance and quality criteria, including access, quality of care, veteran satisfaction, and coordination of care.
- Staffing must include a sufficient number of primary care providers and support staff according to VA standards. Each primary care provider should manage a caseload ratio that meets VA guidelines (1200 active patients per physician; 900 per mid-level provider).
- The contractor must implement a transition plan to ensure continuity of operations upon contract award.
- The facility must be located within 10 miles of a local hospital providing emergency services.

Services will be performed at the contractor's clinic facility located in Dyersburg, Tennessee.
