Description

The Home Oxygen Services contractor will provide a comprehensive Home Oxygen program furnishing all non-personal services, personnel, equipment, tools, supplies, vehicles, training, reports, and facilities necessary for the provision of Home Oxygen services for eligible patients (Veterans) of the VISN 17 area. Performance of this contract will involve interaction with Veteran patients and their Caregivers. Contractor personnel will also interact with the COR and other members of the VISN 17 Prosthetics and Sensory Aid Services (PSAS) and Respiratory Therapy Department during the patient s Home Oxygen therapy continuum of care. This contract may be impacted by changes in regulation, legislation, or medical service policy initiatives identified by VA national, regional, or local directors. For each listed VA Health Care System (HCS), contractor shall provide setup, installation, and management of Home Oxygen and/or Ventilation services to any Patients that are treated at the main VA Medical Center (VAMC) and all Community Based Outpatient Clinics (CBOCs) and Outpatient Clinics (OCs) supported therein (Clinic of Jurisdiction/Primary Service Area COJ/PSA), regardless of their physical location. The listing of counties in the PSA for each VA HCS does not exclude the provision of services in other counties. Place of Performance: VA South Texas Health Care System 7400 Merton Minter Blvd. San Antonio, Texas 78229 South Texas facilities are comprised of the following (46) Texas Counties: Atascosa, Bandera, Bee, Bell, Bexar, Blanco, Brooks, Comal, Concho, Crockett, De Witt, Dimmit, Edwards, Frio, Gillespie, Goliad, Gonzales, Guadalupe, Hays, Jackson, Karnes, Kendall, Kerr, Kimble, Kinney, La Salle, Lavaca, Live Oak, Maverick, McCulloch, McMullen, Mason, Matagorda, Medina, Menard, Pecos, Real, Schleicher, Sutton, Terrell, Uvalde, Val Verde, Victoria, Wharton, Wilson, Zavala.
