Amendment to Solicitation No. 36C25625Q0939 This amendment is issued to incorporate FAR Clause 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, which was inadvertently omitted from the original solicitation. As a result of this addition, this solicitation closing date is extended to July 22, 2025, at 3:00 PM CST to allow all interested parties adequate time to review the clause and prepare their responses accordingly. All other terms and conditions remain unchanged. Please read solicitation carefully and submit all required documentation with proposals.

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with format in Subpart 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Item. This solicitation is issued under authority of FAR 13, Simplified Procedures for Certain Commercial Items, and the conduct of the procurement will be in accordance with FAR Parts 12 and 13 or some combination thereof. The solicitation number is 36C25625Q0939_1 and is issued as a Request for Quotation (RFQ). The proposed contract action is for Durable/Home Medical Equipment/Accessories for which the Government intends to solicit competitive quotes. Interested persons may submit quotes, and all eligible quotes must be received prior to the response date listed on FBO to be considered by the Government. This solicitation is 100% set aside for Small Businesses. Applicable NAICS: 621610 This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC 2023-01). Only emailed requests received directly from the requester will be accepted. Fully licensed company shall provide for the New Orleans VA Medical Center, 2400 Canal St. New Orleans, LA RESPONSES ARE DUE Thursday, June 26, 2025, by 3:00 p.m. CD. Quotes are due electronically to kimberly.anderson6@va.gov no later than the response date listed on FBO. Only electronic quotes will be considered. Phone calls will not be accepted, all question and or concerns must be submitted via email to kimberly.anderson6@va.gov by 1:00pm cst June 23, 2025 by 12:00 p.m.! Background
The Southeast Louisiana Veterans Healthcare System (SLVHCS), part of the Veterans Affairs (VA) network, is seeking to procure Durable/Home Medical Equipment/Accessories (DME/HME) services. The goal of this contract is to provide necessary medical equipment and support services to eligible Veterans and their caregivers, ensuring they receive appropriate care in their homes or designated locations. This solicitation is set aside for small businesses and aims to enhance the quality of life for Veterans by facilitating access to essential medical equipment.

Work Details
The contractor will provide a range of Durable/Home Medical Equipment/Accessories, including but not limited to: hospital beds, over-bed tables, bathroom equipment, invalid lifts, hand cycles, stair glides, room air conditioners, standing tables, portable ramps, mobility aids, and environmental control units. The contractor will also be responsible for delivery and pick-up services, storage, set-up, assembly, disassembly, sanitizing, repair, and maintenance of VA-owned DME/HME. Specific requirements include:

1. Delivery and pick-up of equipment as specified by the Contracting Officer Representative (COR).
2. Repair services for mobility devices such as manual wheelchairs and power wheelchairs.
3. Compliance with The Joint Commission standards for home care.
4. Emergency setup and management of DME/HME services as required by the facility.
5. Maintenance and refurbishing of DME/HME according to manufacturer instructions.

Period of Performance
The contract will have an ordering period spanning five years:

- Year 1: 06/30/2025 - 06/29/2026
- Year 2: 06/30/2026 - 06/29/2027
- Year 3: 06/30/2027 - 06/29/2028

Services will be performed primarily at the Southeast Louisiana Veterans Healthcare System located at 2400 Canal St., New Orleans, LA. Additional service areas include various Community Based Outpatient Clinics (CBOCs) across Louisiana.
