This is an amendment to provide the Government response to submitted follow up questions, update attachment D.4 Validated Peripheral List and extend the proposal submission date for the contracted Community Based Outpatient Clinic (CBOC) for Primary Care services for the Oklahoma City VA Health Care System (OKCVAHCS) to be located within the city limits of Altus, OK. As a result, the close date is extended to 4/23/2024, 10:00 AM MDT.

Background
The contract is for the provision of Community Based Outpatient Clinic (CBOC) services within Altus, Oklahoma, under the authority of 38 U.S. Code § 8153 - Sharing of health-care resources. The period of performance will be one (1) base year with nine (9) twelve (12) month option periods.

Work Details
The Contractor shall provide Primary Care CBOC services including medical and mental health care, support services such as pharmacy, laboratory, and x-ray. The Contractor is required to meet VHA performance and quality criteria and standards including access, customer satisfaction, prevention index, chronic disease index, and clinical guidelines. The Contractor shall provide PACT staffing in numbers and qualifications capable of fulfilling the standards outlined in the resultant contract.
The Contractor shall also establish and implement contingency plans for ensuring patients receive continuity of and access to appropriate primary care during periods of inadequate resources, extended staff absences, staff turnover, understaffing, and nature-related events.

Period of Performance
The period of performance will be one (1) base year with nine (9) twelve (12) month option periods.

Place of Performance
The geographic location for the contract performance is within the city limits of Altus, Oklahoma.
