Alexandria Prosthetics DME Contract Amend 002

This is a Set-Aside Small Business (SB) Request for Quote (RFQ). The government anticipates awarding a firm-fixed price contract resulting from this solicitation. All respondents must have an active registration in the System for Award Management (SAM) at www.sam.gov. Interested SDVOSBs must be listed and certified at SBA Veteran Small Business Certification (VetCert) https://veterans.certify.sba.gov/ to receive contract awards. Department of Veterans Affairs, Network Contracting Office 16, is soliciting offers for Durable Home Medical Equipment Accessories. The Contractor shall provide Durable Home Medical Equipment Accessories (DME/HME) Services in accordance with the details outlined in the Performance Work Statement (PWS). Work will be performed for the Alexandria VA HealthCare System, 2495 Shreveport Hwy 71, Pineville, LA, 71360. Background
The Alexandria Veterans Health Care System, part of the VISN 16 VA Healthcare Network, is seeking a contractor to provide Durable Home Medical Equipment Accessories (DME/HME) Services. The goal of this contract is to ensure that all necessary labor, facilities, pick-up and delivery, set-up, storage, repairs, and inventory management of VA-owned equipment are effectively managed for eligible Veterans and their caregivers.

Work Details
The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision and non-personal services necessary to deliver Durable Medical Equipment (DME) supplies. Specific tasks include:
1. Delivery and pick-up of DME/HME to eligible Veterans/Caregivers.
2. Receiving and storage of DME/HME.
3. Set-up and assembly of DME/HME at the beneficiary's location.
4. Disassembly and take-down of equipment when no longer needed.
5. Providing instructions for use and care of the equipment.
6. Sanitizing and maintaining the equipment as per health standards.
7. Repair services for mobility devices including manual wheelchairs, power chairs, and scooters.

Period of Performance
The contract will be effective from November 14, 2025 to November 13, 2030 with five ordering periods:
- Ordering Period One: November 14, 2025 – November 13, 2026
- Ordering Period Two: November 14, 2026 – November 13, 2027
- Ordering Period Three: November 14, 2027 – November 13, 2028
- Ordering Period Four: November 14, 2028 – November 13, 2029
- Ordering Period Five: November 14, 2029 – November 13, 2030.

Place of Performance
Work will be performed at the Alexandria VA HealthCare System located at 2495 Shreveport Hwy 71 Pineville, LA 71360.
