Background
The Department of Veterans Affairs (VA) San Diego Healthcare System (VASDHS) is seeking to establish a contract for primary care services for Veterans in Escondido, California. This initiative aims to enhance access to healthcare for Veterans through a Community-Based Outpatient Clinic (CBOC), aligning with the VA's mission to provide high-quality care and support to those who have served in the military.

Work Details
The contractor will be responsible for providing comprehensive primary care services at the Escondido CBOC, which includes:

1. Delivering a continuum of care from prevention to diagnosis and treatment, including appropriate referrals and follow-ups.

2. Ensuring compliance with VHA performance and quality criteria, including access, quality of care, Veteran satisfaction, and clinical guidelines.

3. Staffing requirements include a Patient Aligned Care Team (PACT) consisting of primary care providers, registered nurse care managers, clinical associates, and administrative associates.

4. The contractor must maintain a sufficient number of primary care providers to meet VA standards of patient load ratios.

5. The contractor will handle laboratory services, radiology requests, and other ancillary services as specified in the Performance Work Statement (PWS).

6. The contract includes provisions for billing procedures based on a per-member-per-month (PMPM) model for an estimated number of billable patients.

Period of Performance
The contract will have a base period of one year starting from March 5, 2025, with nine additional option years available.

Place of Performance
The services will be performed at the Community-Based Outpatient Clinic located within the city limits of Escondido, California.
