SOURCES SOUGHT: Veterans Health Affairs Network Contracting Office 4 (NCO4) is issuing this request for information/sources sought notice in order to identify contractors that can provide dental laboratory services for the Dental Clinic at VAPHS Heinz Campus, located at VAPHS, H. J. Heinz Division, 1010 Delafield Road, Pittsburgh, PA 15215. SERVICES TO BE PROVIDED: A Statement of Work (SOW) is attached to this Sources Sought Notice. The SOW details the requirement. A list of required items/services is included in the Statement of Work, which is attached to this Sources Sought Notice (Attachment 1 Statement of Work). The estimated required quantities of the listed items/services are also included in the SOW. Note: The commercial dental laboratory must be able to provide pick-up/delivery services to the Dental Clinic at VAPHS Heinz Campus in approximately 1 hour of request, allowing for such variables as traffic and weather. The dental laboratory must provide courier service for pick-up and delivery of case work from the Dental Clinic at the VAPHS Heinz Campus. The Dental Clinic is located at VAPHS, H. J. Heinz Division, 1010 Delafield Road, Pittsburgh, PA 15215. The proximity allows for same day turnaround time on repair cases. The proximity also allows for personal attention on specific cases. VAPHS anticipates that daily inside pick-ups and/or inside deliveries will be required. Partnerships and subcontracting to meet the requirement is permitted. However, FAR 52.219-14 applies, which states that at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. As such, the prime contractor must be able to perform at least 50% of the required work. All proposed partnerships and subcontracting relationships should be discussed in the capability statement.
