SOURCES SOUGHT: Veterans Health Affairs Network Contracting Office 4 (NCO4) is issuing this request for information/sources sought notice in order to identify contractors that can provide dental laboratory services for the Lebanon VA Medical Center (LVAMC), located at 1700 S. Lincoln Ave., Lebanon, PA 17042. SERVICES TO BE PROVIDED: A Statement of Work (SOW) is attached to this Sources Sought Notice. The SOW details the requirement. A list of required items/services is included in the Statement of Work, which is attached to this Sources Sought Notice (Attachment 1 Statement of Work). The estimated required quantities of the listed items/services are also included in the SOW. Note: Finished products must be delivered to LVAMC within seven (7) calendar days of order notification, except for repair, reline or rebases which require delivery within three (3) days of notification. Partnerships and subcontracting to meet the requirement is permitted. However, FAR 52.219-14 applies, which states that at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. As such, the prime contractor must be able to perform at least 50% of the required work. All proposed partnerships and subcontracting relationships should be discussed in the capability statement. The North American Industry Classification System Code (NAICS) is 339116 and the small business size standard is 500 Employees. RESPONSES: NCO 4 is seeking responses from contractors that are interested in this requirement and can demonstrate their capability to provide the required services. This must include a statement explaining your capability to service the Lebanon VA, ability to provide all items listed in the Statement of Work, and ability to provide pick-up/delivery dental services to the VA. If you are unable to provide all requirements, indicate specifically which requirements you are unable to meet, and provide an alternative solution.
