Background
The Indian Health Service - Warm Springs Health and Wellness Center (WSSU), Oregon 97761, plans to acquire propane gas delivery services. The purpose of this combined synopsis and solicitation is for the purchase and delivery of Propane Gas IAW the attached Statement of Work (attachment 1). The contract will be awarded based on Lowest Price Technically Acceptable.

Work Details
The contractor is required to provide propane gas service to the Warm Springs Health and Wellness Center. This includes providing all labor, materials, tools, equipment, and transportation for propane delivery to the service locations. Propane shall be provided on an as-needed basis for five propane tanks at the location. The contractor is responsible for coordinating propane services with the point of contact for delivery, delivering and providing propane in compliance with federal, state, local, tribal, and IHS regulations, conducting annual leak testing, providing a receipt for delivery, quantity, and price, reading existing individual meters and billing/collecting payment from housing tenants for actual use of propane.

Period of Performance
The period of performance includes the base year from 8/14/2022 to 8/13/2023 and option years from 8/14/2023 to 8/13/2027.

Place of Performance
Propane delivery will be provided to Warm Springs Health and Wellness Center located at 1270 Kot-Num Rd, Warm Springs, Oregon 97761.
