Defense Health Agency (DHA) has a requirement for a Firm Fixed Price (FFP) IDIQ supply contract in accordance with FAR Part 12, Commercial Items and FAR Part 13, Simplified Acquisition Procedures for Pre-Filled Syringes and Bags for the Pharmacy Department at Naval Medical Center Portsmouth.

In order to be considered for this requirement, please respond with a completed proposal following the guidelines in the Instructions to Offerors (page 45) and Evaluation Factors For Award (page 48) of the SF1449.

The amended solicitation HT940625QE012 0001 corrects Attachment 2- Past Performance Questionnaire where the title of the requirement was incorrect and it has been corrected to IDIQ Pre-filled Syringes and Bags at the Naval Medical Center Portsmouth.

Background
The Defense Health Agency (DHA) is issuing a requirement for a Firm Fixed Price (FFP) IDIQ supply contract for pre-filled syringes and bags to support the Pharmacy Department at Naval Medical Center Portsmouth.

This contract aims to ensure the continued provision of essential pharmaceutical supplies to military families, as the current contract is set to expire on September 30, 2025.

Work Details
The contractor shall provide pre-filled syringes and bags, including but not limited to the following FDA approved medications:
- Fagron Glycopyrrolate 0.2mg/mL (300 units)
- Ephedrine Sulfate 5mg/mL (200 units)
- Hydromorphone HCl 0.2mg/mL (1,000 units)
- Ketamine 10mg/mL (864 units)
- Neostigmine Methylsulfate 1mg/mL (580 units)

The contractor is responsible for all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary for performance.

Deliveries will be made on an as-needed basis to the Naval Medical Center Portsmouth.

Period of Performance
Base period from October 1, 2025 to September 30, 2026 with four option years extending through September 30, 2030.
