Description

Contractor personnel shall administer surveys ( via phone and via mailed surveys) to the next of kin of deceased inpatient Veterans for the purpose of collecting and documenting this data for VA's Hospice and Palliative Care program as a central indicator of the quality of care delivered. The feedback from these surveys will provide the "voice of veterans" in VA's strategic planning to be a patient-centered healthcare organization. Place of Performance: Services shall be provided on site at the Lebanon VA Medical Center, 1700 S. Lincoln Ave, Lebanon, PA 17042.
