Background
This solicitation is issued by the Crownpoint Health Care Facility, which is part of the Indian Health Service. The mission of the facility is to provide comprehensive health care services to the Navajo Nation and surrounding communities. The goal of this contract is to secure Non-personal Nurse Practitioner Services to support the Medical Staff Department at the facility, ensuring that quality healthcare is delivered to patients in a timely manner.

Work Details
The contractor shall provide Non-personal Nurse Practitioner (NP) Services for the Medical Staff Department at the Crownpoint Healthcare Facility. Services include:
1. Providing NP services in various locations including the Emergency Room, Adult Ambulatory Care Clinic, Thoreau Health Clinic, and Pueblo Pintado Clinic.
2. Following work hours specified in the Crownpoint Medical Staff schedule, with shifts typically ranging from 10-12 hours.
3. Flexibility in clinic hours with typical operating hours from 7:30 a.m. to 6:00 p.m. or 8:00 a.m. to 4:30 p.m.
4. No on-call coverage required for evenings, weekends, or federal holidays.
5. Timekeeping and attendance will be monitored in compliance with contract terms.
6. The contractor will operate as an independent contractor and must adhere to all applicable regulations and standards outlined in the Performance Work Statement (PWS).

Period of Performance
The period of performance for this contract is from October 1, 2025, to September 30, 2026, with an option for an additional year extending from October 1, 2026, to September 30, 2027.

Place of Performance
The services will be performed at various locations within the Crownpoint Healthcare Facility and surrounding clinics located in Crownpoint, NM.
