Amendment 0003 This amendment reflects clarification to questions pertaining to RFP 36C26223R0036 0002 Home Medical Equipment (HME) Services for the VA Southern Arizona Healthcare System. Please see below for the questions and answers. Question 1: Can you provide the previous award amount? Answer 1: The previous award amount was $1,046,750.00 for the VA Phoenix Healthcare System ( Base + 4 Option Years ). Please the following questions below for the name of the incumbent and the contract number. Question 2: Can you please provide the incumbent? Answer 2: The previous incumbent for Home Medical Equipment Services was AAA Mobility Specialist, LLC. Question 3: What is the contract number? Answer 3: The previous contract number for the AAA Mobility Specialist LLC is 36C26222D0022. Question 4: At present, what zip codes have the highest concentration of veterans that require these services? Answer 4: We do not differentiate veterans by zip codes, they are counted by facility only. We have already provided the estimated quantities in the Price Worksheet attachment, which was included in the original solicitation posting.
