Multi Part-time General Dentist Services

The RFP is issued as a 100% Set-Aside Small Business concerns and only quotes from qualified offerors will be considered under the associated NAICS Code is 339116, which is small business standard size 500.

The anticipated Period of Performance for these services is July 1, 2018 through June 30, 2019 with one, 1-year option.

The Albuquerque Area Indian Health Service (AAO) intends to award a firm-fixed price, non-personal service per FAR 37.4 - Nonpersonal Health Care Services, Purchase order (PO), commercial item in response to Request for Proposal (RFP) 18-242-SOL-00038.

This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1

The daily price must be all inclusive (all necessary labor, materials, equipment, supplies, permits, licenses, New Mexico Gross Receipt Tax, and transportation costs to provide general dental services) plus all other cost pertinent to the performance of this contract. Utilize your most competitive and reasonable rates.

The contractor will be required to perform for 9 hours per day(clinic day), 2 days a week, 52 weeks a year at the Albuquerque Indian Dental Clinic. The fee is a flat per-day rate with up to 15 - 25 patients scheduled each day. The contracts will be awarded as a single fixed price requirement contract for base year and one one-year option for renewal. The contractor must be certified by the American Board of General Dentistry and licensed in a state of the United States or its territories in order to practice general dentistry. All interested offerors must meet the evaluation factors stated in the solicitation.
