**Amendment 0004 is to address the follwing:

VA has been made award of a formatting issue on Attachment M and is providing the following instructions:

Offerors may create an addendum to Attachment M that serves in place of any section(s) with formatting issues. The addendum must state which section it replaces, for example - "This addendum to Attachment M is for Past Performance Number 3" etc. Offerors shall clearly mark attachments as being apart of Attachment M. Please note that all sections of Attachment M are still required. Use the procedure above if affected.

**Amenedment 0003 is to answer the follwing:

Would a small business that subcontracts with a SDVOSB subcontractor be able to obtain partial credit and/or consideration in the Socio-Economic evaluation? Answer: Yes.

What is the percentage of work that must be performed by the SDVOSB Small Business in order to achieve contract compliance and/or consideration in this evaluation factor. Answer: Refer to Q/A. 29 for percentages.

**Amendment 0001 is to provide Government responses to vendor questions. Please see the attached Questions and Answers, amended Solicitation, and Attachment M. All changes are highlighted in yellow.

Overview: The Department of Veterans Affairs (VA) is seeking to establish multiple Blanket Purchase Agreements (BPAs) for services under the North American Industry Classification System (NAICS) code 813920 and Product Service Code (PSC) R499. The government will utilize the FAR 13.5 Simplified Acquisition Procedures for this procurement. This solicitation is issued on an unrestricted basis, allowing for full and open competition. All responsible sources may submit a proposal which shall be considered by the agency.

Scope: The Contractor shall provide external expertise and conduct a peer review of selected episodes of care. The Contractor shall use the provided platform (Electronic Health Record) for the review of medical records and report templates.

Under this Blanket Purchase Agreement (BPA), the Contractor shall perform the following:

Peer Review for Quality Management

o Audits

o Facility Request Reviews

Management Review

o Management Review

o Clinical Appeals

Medical Advisory Opinions

Executive Summaries

Reports

Inter-Rater Reliability (IRR) Studies

Performance Period: The BPAs will be established for a period of performance of five years.
