Colville Service Unit (Omak & Nespelem) Dental X-ray equipment Qty Four (4)
The Indian Health Service (IHS) is conducting a SOURCES SOUGHT to identify potential vendors capable of providing Midmark 5415 Preva x-ray equipment for the Colville Service Unit, two delivered to the Nespelem address and two delivered to the Omak location.
This is NOT a solicitation for proposals, proposal abstracts, or quotations.
The Government is seeking to identify companies capable of providing the dental x-ray equipment to the Colville Service Unit. (Omak & Nespelem locations) Any vendor responding to this notice should submit their capability statement. Instructions for submission are outlined below
The purpose of this notice is to obtain information regarding: (1) the availability and capability of all qualified native small businesses, small businesses, or large businesses: (2) whether they are: HUBZone small businesses: service-disable, veteran owned small businesses; (8a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification according to the North American Industry Classification System (NAICS), 339114 code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. This notice is in accordance with 25 USC 14, Buy Indian Act with preference to be given to Indian Small Business Economic Enterprises (ISBEE) and/or Indian Economic Enterprises (IEE).
Interested parties shall not be reimbursed for costs associated with preparation of their responses. Any proprietary information should be marked. Interested organizations presenting a Capability Statement in response to this Sources Sought Notice must identify their size status.
Respondents must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application.
Responses should include: (1) Business Name, (2) Point of Contact Name, Phone Number, and E-mail Address, (3) UEI, and NAICS code, (4) Business Size SMALL OR LARGE, (5) Type of Business NATIVE OWNED, SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED, etc., (6) Demonstration capability, and capacity to furnish services. All capability statements sent in response to this Sources Sought Notice must be submitted electronically (via email) to the Point of Contact below in MS Word or Adobe Portable Document Format (PDF). The Government requests Capability Statements from qualified offerors demonstrating their ability to deliver the dental x-ray equipment. Interested parties shall submit written responses to this notice, which includes:
- Core Competencies: Brief summary of expertise and capabilities as it relates to the agency's needs.
- A capability statement that addresses the company's qualifications and ability to provide the dental x-ray equipment, include contract numbers, description, dollar value, customer point(s) of contact with current telephone number and/or email address.
- Documentation that your company is an authorized distributor or reseller for the Midmark dental x-ray equipment.
- Confirm your Buy Indian set-aside status by submitting the attached IHS Buy Indian Act Indian Firm Representation form, if applicable.
- Company data: Company name, physical address, telephone and fax numbers, website, Taxpayer Identification Number (TIN), SAM Unique Entity Identifier (UEI) code, NAICS codes and Business Size Classification (e.g., 8(a), HUBZone, etc.), primary point of contact name, telephone number and email address.
All capability statements sent in response to this Sources Sought notice must be submitted electronically (via email) to the Point of Contact below in MS Word or Adobe Portable Document Format (PDF). The subject line must specify the Sources Sought Number and Title. CAPABILITY STATEMENTS RECEIVED AFTER THIS DATE AND TIME WILL NOT BE CONSIDERED. If adequate interest is not received from Buy Indian concerns, a solicitation may be issued as another type of set-aside or unrestricted, without further notice.
