Male Homeless Shelter

Background
The Department of Veterans Affairs (VA) is issuing this solicitation for the Health Care for Homeless Veterans (HCHV) program, which provides essential case management services to homeless male Veterans. The program aims to facilitate access to a wide range of medical, substance abuse, mental health, rehabilitative, and employment services.

The goal is to engage homeless male Veterans in supportive services and place them in safe environments that support their rehabilitation and reintegration into the community. This contract is part of a broader initiative under 38 U.S.C. 2031 to provide housing and support services to homeless Veterans.

Work Details
The contractor will provide residential treatment services to at least five homeless male Veterans with substance abuse or mental health diagnoses authorized by the Atlanta VA Health Care System (AVAHCS). Services include:
1. Clean, sanitary, and safe residential housing.
2. Mental health and substance abuse case management.
3. Structured group activities including psycho-therapeutic and physical activities.
4. Instruction in health and personal hygiene.
5. Medication monitoring with appropriate documentation and secure storage.
6. Transportation for medical appointments and group activities as needed.
7. Routine breathalyzer and urine drug screening as required.
8. Employment placement and job training assistance.
9. Placement of Veterans into permanent affordable housing after transitioning out of the program.

Period of Performance
The contract will have a performance period of five years.

Place of Performance
Services will be performed at the contractor's facility located within a 15-mile radius of the VA at 1670 Clairmont Rd., Decatur, Georgia, 30033.
