Description

  1. Offeror shall provide a 1 Year comprehensive coverage for maintenance and service of two GE Defiunim 656 rooms, two GE Optima 240 portable x-ray and six imaging detectors with 4 option years at a fixed annual service cost that includes all of the following:
    1. OEM Planned Maintenance Service per OEM specifications
    2. OEM Routine System Calibration Tests
    3. OEM Quality Assurance Evaluations
    4. Table repairs
    5. Unscheduled repair visits to correct equipment malfunctions, which includes OEM replacement parts, travel, and labor. Unscheduled repairs shall have a 6 hour on-site response time. Tube coverage, detector coverage and battery replacement included.
    6. Grid replacement
    7. Repair Parts are included, next day delivery by 10:30 am.
    8. Software and quality updates to enhance the performance of the equipment as recommended and issued by GE Healthcare
    9. TiP Answer Line
    10. Offeror shall provide Technical Support to include unlimited calls for customer and service support. Phone support shall have a 30 minute response time.
    11. ILINQ Response time 30 MIN
    12. 97% Uptime Guarantee
    13. Normal business hours coverage M-F 8AM 5PM (CST) excluding Federal Holidays
  2. Offeror shall submit a service report, and checklist if applicable, to the Lawton Service Unit Biomedical Engineering Department within five business days after services rendered.
  3. Offeror shall notify the Lawton Indian Hospital Biomedical Engineering Department prior to performing any services.
  4. Offeror shall sign in/out at the Security Office upon enter/exiting facility for service.
  5. Offeror shall remit and original and two (2) copies of its invoice to:
Background
