Background
The Department of Veterans Affairs is issuing this Request for Quote (RFQ) to provide Medical Waste Removal Services for the Robley Rex VA Medical Center. The goal of this contract is to establish a Blanket Purchase Agreement (BPA) that will facilitate the safe and compliant disposal of medical waste, ensuring the health and safety of patients and staff at the facility. This RFQ is set aside 100% for Service-Disabled Veteran-Owned Small Business (SDVOSB) contractors.

Work Details
The contractor will be responsible for the following tasks:
1. Remove and dispose of Trace Chemotherapy Waste, with a quantity of 442 units per order.
2. Remove and dispose of MORGUE Regulated Medical Waste, with a quantity of 5 units per order.
3. Remove and dispose of Regulated Medical Waste (RMW) from multiple locations including Greenwood CBOC, Newburg CBOC, Carrollton CBOC, Scottsburg CBOC, New Albany CBOC, Grayson CBOC, Fort Knox CBOC, Lagrange CBOC, Elizabethtown CBOC, and New Albany OPC (future clinic), with a quantity of 364 units per order.

The BPA will consist of a one-year ordering period followed by two additional one-year ordering periods. The contractor must develop and maintain a written safety plan to protect staff and the surrounding community during waste removal operations.

Period of Performance
The period of performance for this contract is from September 1, 2025, through August 31, 2028. This includes a base ordering period from September 1, 2025, to August 31, 2026; an option period from September 1, 2026, to August 31, 2027; and a second option period from September 1, 2027, to August 31, 2028.

Place of Performance
The services will be performed at the Robley Rex VA Medical Center located at 800 Zorn Ave., Louisville KY 40206 and various Community-Based Outpatient Clinics (CBOCs) in the surrounding areas.
