On behalf of the Lexington VA Health Care System, Network Contracting Office (NCO) 9 is issuing this Request for Quote (RFQ) IAW FAR Part 13. This RFQ will result in a Blanket Purchase Agreement (BPA) with a period of performance of 05/01/2025 through 04/30/2028. The BPA will consis of 3 (three) one-year ordering periods which will allow for price escalation each year.

QUOTES ARE DUE BY 4:00 PM CT ON APRIL 29, 2025.

AMENDMENT 1 IS QUESTIONS AND ANSWERS 04/25/2025.

AMENDMENT 2 IS TO EXTEND THE REQUEST FOR QUOTE UNTIL 4:00 PM CT MONDAY, MAY 5, 2025. QUESTIONS ARE DUE BY 12:00 PM CT ON THURSDAY, MAY 1, 2025.

AMENDMENT 3 IS TO EXTEND THE REQUEST FOR QUOTE UNTIL 4:00 PM CT WEDNESDAY, MAY 14, 2025, SHARE QUESTIONS AND ANSWERS, AND REMOVE THE PERIOD OF PERFORMANCE FROM THE STATEMENT OF WORK. It is advised that interested parties conduct site visits to obtain all necessary information to provide accurate quotes. Site visits can be arranged by sending an email to Rachel Hall at Rachel.Hally7@va.gov and Kimberly Coleman at KimberlyJ.Coleman2@va.gov.

AMENDMENT 4 IS TO EXTEND THE REQUEST FOR QUOTE UNTIL 12:00 PM CT WEDNESDAY, MAY 21, 2025 DUE TO THE UNAVAILABILITY OF THE CORS TO CONDUCT SITE VISITS ON MAY 6, 2025 THROUGH MAY 8, 2025.
