Description

Veterans Integrated Service Network (VISN) 21 requires access to specialty and esoteric reference laboratory testing services for tests not performed in-house. VISN 21 is comprised of the following facilities: VA Pacific Islands Healthcare System (VAPIHCS), VA Northern California Healthcare System (VANCHCS), VA Palo Alto Healthcare System (VAPAHCS), San Francisco VA Medical Center (SFVAMC), VA Central California Healthcare System (VACCHCS), VA Sierra Nevada Healthcare System (VASNHCS), and VA Las Vegas Healthcare System (VALVHCS) (hereinafter each generically referred to as "VAMC"). Provide necessary supplies including but not limited to requisition forms, specimen containers, special instructions, specimen carriers, all forms (including chain of custody forms), special media or tubes for viral samples, all supplies for special tests (eg. viral studies), printers and printer supplies, middleware, software. The VAMCs will be responsible for collecting the specimens, including processing the specimens. The referral laboratory will be responsible for packaging the specimens for shipment, including ensuring that the samples received are transported in an appropriate and controlled environment by packing them into the appropriate containers. The contractor is responsible for providing all supplies, equipment, packaging materials, cooling materials, etc. and retrieving the individual test specimens from the VAMC laboratories and packing them into appropriate containers/coolers/boxes for shipment. Transport personnel employees should adhere to CAP GEN.40515. Laboratories using third-party private couriers are responsible for ensuring that laboratory specimens are packaged and transported properly. The RFQ closes on June 24, 2020 at 1:00 PM PT. GSA E-Buy does not allow me to leave a solicitation open this long. The GSA Ebuy close date is 5/29/20. However, an amendment will be posted before this date to extend the deadline to June 24, 2020.
