See RFQ and attached documents. In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in VetCert database if applicable (VetCert registration is required for Service-Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Contractors may obtain SAM information at https://www.sam.gov and VetCert information at https://veterans.certify.sba.gov/#home. To be eligible for award, contractors must be registered in the System for Award Management and VetCert databases. Registration is free and potential offerors are encouraged to visit the SAM website as no contracts can be awarded to any offeror not registered in SAM.

Background
The Department of Veterans Affairs (VA) is issuing a solicitation for a Blanket Purchase Agreement (BPA) to provide Surgical Scope Repair Services for the Bay Pines VA Healthcare System. This initiative aims to ensure that functional surgical instrumentation is available for patient care, aligning with the VA's mission to provide high-quality healthcare services to veterans. Work Details
The contractor will be responsible for the repair, refurbishment, and assessment of surgical devices including cystoscopes, RHINO-laryngoscopes, ureteroscopes, duodenoscopes, cystonephro fiberscopes, cystonephro videoscopes, telescopes (0, 30, 45, 70 degrees), laparoscopes (all sizes), arthroscopes, and HD camera heads. Key tasks include:

- Picking up devices within two business days after notification and providing loaner replacements.
- Delivering repaired devices back within seven business days.
- Conducting semi-annual preventive maintenance on serialized devices.
- Implementing a tracking system for device status during repair and shipping.
- Ensuring compliance with FDA and other regulatory standards regarding surgical instrument repair.
- Providing documentation of quality assurance per VHA Directive 1116 including verification of device completeness and functionality.
- Offering training programs approved by the Department of Labor for instrument technicians.
- Maintaining an Alternative Equipment Maintenance (AEM) program tailored to facility needs.
- Reporting on non-conforming devices found during inspections and providing corrective action recommendations.

Base Year: September 1, 2025 – August 31, 2026;
Option Year One: September 1, 2026 – August 31, 2027;
Option Year Two: September 1, 2027 – August 31, 2028;
