Statement of Work Background: The White River Junction VA Medical Center Sterile Processing Services (SPS) Department has purchased Air Innovation Sterile Storage cabinet is designed to store and protect Reusable Medical Devices (RMD). The cabinets will require yearly maintenance to ensure the optimal working parameters such as: internal chamber volume will be maintained at the following conditions: 66 -72 F, 20-60% RH (max dew point 57 F), and a minimum internal positive pressure + 0.03 In-H20 or greater to maintain ISO Class 5 conditions, ambient temperature ranges from 60-82 F and 10%-50% RH. The cabinets have specialized sensors that would need to be recalibrated, as of right now, there is no one in White River Junction that has been certified to perform the delicate work that is necessary to keep the cabinets within proper parameters. Having the Air Innovation team come and do all the work would cut down on the need of training multiple people on these cabinets as well as ensure that all responsibility for the PMs do not fall on the facility. The cabinets are necessary to maintain the sterility of the instruments as the temperatures fluctuate and our aging facility struggles to keep proper environmental controls. Air Innovation Sterile Storage Cabinet Preventative Maintenance will serve the following primary functions: Replace HEPA Filter Annually, HEPA Filter included. Check Airflow, Check Filter Integrity, Check Static Pressure Replace Humidifier water reservoir, water pump, all hoses and tubing associated with Humidifier water distribution annually, all parts Included. Test all tubing for leaks, hose clamps and operational integrity. Check Humidifier operation Inspect heater element & Liquid level Sensor, clean water level controller level sensors and test. Check Thermoelectric cooler & clean. Current, voltage, temperature, and fans. Check Electric Heater. Current, voltage, temperature rise, clean. Check Recirculation fans: Current, voltage, and fan performance, clean. Check Heat rejection Cabinet fans: Current, voltage, and fan performance, clean Check Cabinet Pressure controller and calibrate as needed. Check Temperature, Humidity & Pressure Sensor Calibration. Check all electrical cords and connections. Check door seals and latching mechanisms. Check all gasketing integrity, repair as needed. Clean Cabinet and leave a copy of PM service report with Customer. Place of Performance/Delivery Location: White River Junction VAMC: 215 North Main Street White River Junction, VT 05009 Vendor Responsibilities: Implementation services shall include the following: On-site installation coordination of all equipment and accessories. Meetings should be scheduled with site for implementation, initial deployment, and through the completion of the delivery. Materials shall be new equipment, parts, and accessories as specified in the purchase order. The vendor shall have the materials delivered to the job site in original equipment manufacturer (OEM) original unopened containers, clearly labeled with the OEM s name, equipment model and serial identification numbers, delivery order number, and VA purchase order number. If products do not meet criteria, site POC will contact the Vendor Project Manager for replacement of that equipment. The vendor is responsible for inventorying materials prior to delivery to VA sites to check for accuracy in quantity and part number. Must be verified by VA POC when delivered onsite Vendor shall provide upgrades during the renewal of an option or unless approved by the vendor. Upgrades will be for those pieces of equipment specifically identified by the vendor as they become available. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. Government Responsibilities: Each VA site is responsible for inventorying materials prior to acceptance of order. Each VA site shall identify and provide limited daily storage of vendor parts/supplies at the time of delivery. Each VA site shall reject items that do not conform to requirements above. Equipment shall be checked upon opening the packaging to ensure no damage occurred during shipment. If damage is observed, POC will contact Vendor Project Manager for replacement of that equipment.

Work Details
