Statement of Work (SOW) One time Repair on Philips C10-3V Ultrasound Transducers Department of Veterans Affairs (VA) Veterans Health Administration (VHA) PURPOSE: Veterans Affairs requires a contractor to provide repair services for two Philips C10-3V Ultrasound Transducers. SNs: F0BNXT & F0G4T2. Services are to be performed in accordance with this SOW and to OEM standards, with the intent of ensuring the equipment operates as the original manufacturer intended, in a cost-effective, efficient manner, contributing to the VA Mission. Services are required on the government-owned systems listed in this brief scope of work for the period of performance and at the location outlined in this brief SOW. BACKGROUND: Equipment: The following systems of equipment require service under this contract. This includes sub-systems, components, software, hardware, and parts. Equipment Description Brand & Model Original Equipment Manufacturer (OEM) Serial Number VA PMI Number Philips Ultrasound Transducer Philips C10-3V Ultrasound Transducer Philips F0BNXT 97929 Philips Ultrasound Transducer Philips C10-3V Ultrasound Transducer Philips F0G4T2 99743 Contractor shall provide OEM-trained technicians to perform OEM-standard services and repairs on the equipment above. Services on the equipment should ensure they are in optimal working condition at the conclusion of such repairs. IMPORTANT NOTE: This RFI (Request for Information) is for information only. No award will be made from this RFI. Please communicate via e-mail to orrellius.williams@va.gov by 13:00 on 06/06/2025 as to your company s ability to perform service per this brief Scope of Work. Background
The Department of Veterans Affairs (VA), specifically the Veterans Health Administration (VHA), requires a contractor to provide repair services for two Philips C10-3V Ultrasound Transducers. The goal of this contract is to ensure that the equipment operates as intended by the original manufacturer, in a cost-effective and efficient manner, contributing to the VA's mission.

Work Details
The contractor shall provide OEM-trained technicians to perform OEM-standard services and repairs on the following equipment:

1) Philips C10-3V Ultrasound Transducer, Serial Number F0BNXT, PMI Number 97929;
2) Philips C10-3V Ultrasound Transducer, Serial Number F0G4T2, PMI Number 99743.

Services must ensure that the equipment is in optimal working condition at the conclusion of repairs. The scope includes all necessary sub-systems, components, software, hardware, and parts related to these transducers.

Place of Performance
The services will be performed at the Oklahoma City VA Health Care System located at 921 N.E. 13th St., Oklahoma City, Oklahoma, 73104.
