Page 3 of 3 NOTICE OF INTENT TO SOLE SOURCE Philips Full-Service Contract The Veterans Health Affairs, Network Contracting Office 16, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide maintenance and repair service for Philips Medical imaging equipment and systems to include: radiology, CT, and MR systems as described below in the statement of work. The government is contemplating a Base +4 option period Firm Fixed Price contract to satisfy this requirement and meet the needs of the patient population at the G.V. Contract Title. Service contract for Philips s imaging equipment including radiology, CT, and MR systems. 3. Background. G.V. (Sonny) Montgomery VA Medical Center is requesting the maintenance and repair service for Philips Medical imaging equipment and systems to include radiology, CT, and MR systems. The period of performances for this service contract shall be July 1, 2022 through September 30, 2022, with four option years following for FY 23, 24, 25, and 26. Please execute a contract for FY22 against national contract number VA797H-17-D-0022 from Philips Healthcare. 4. Scope. The equipment to be serviced/repaired/maintained under this agreement is listed in the tables below. All of the equipment listed is owned by the G.V. (Sonny) Montgomery VA Medical Center. Hours of Coverage: A. Normal hours of coverage are Monday through Friday 8am to 5pm. Service will be performed during normal hours of coverage unless requested or approved by COR. B. Federal Holidays observed by the VAMC are: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day Identification, Parking, Smoking, and Security: Contractor FSE shall wear visible identification at all times while on the premises of the GVSMVAMC. It is the responsibility of Contractor FSE to park in the appropriate designated parking areas. Information on parking is available from the VA Police Service. The GVSMVAMC will not invalidate or make reimbursement for parking violations of Contractor s FSE under any conditions. Smoking is prohibited inside any buildings at the GVSMVAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. Place of Performance. Service and maintenance work will be completed on site at the G.V. (Sonny) Montgomery VA Medical Center. Period of Performance. Vendor will provide service for the period July 1, 2022 through September 30, 2022, with four option years following for FY 23, 24, 25, and 26.
