Notice of Intent to Sole Source This is a Notice of Intent to Sole Source the following requirement. Reference #: 512-22-2-098-0092 TITLE OF PROJECT: KARL STORZ ENDOSCOPY SUPPORT TYPE OF CONTRACT: MAINTENANCE SERVICES PURPOSE AND SCOPE OF WORK: The vendor will provide support for the Karl Storz endoscopy equipment and instrumentation currently in use at the Baltimore VA Medical Center. Support will include unlimited repairs and exchanges for the covered products, including pre-paid shipping, parts, and labor, as well as 24/7/365 technical service support. All repairs must return the equipment to the device s original specifications. The vendor will supply the Baltimore VAMC Biomedical Engineering department with a copy of a detailed service report that meets the requirements of 21CFR820, subpart N, within 2 weeks of a repair being completed. All exchanges must be documented with an RMA provided to the Baltimore VAMC Biomedical Engineering department. This service coverage includes unlimited repairs and exchanges for the covered products that return the equipment to the original specification to ensure the safety and performance of the covered equipment as well as the efficacy of the documented reprocessing procedures. ORDERING PERIODS: Base Year: 02/01/2022 through 01/31/2023 Ordering Year One: 02/01/2023 through 01/31/2024 Ordering Year Two: 02/01/2024 through 01/31/2025 PLACE OF PERFORMANCE: Baltimore VA Medical Center 10 N. Greene St Baltimore, MD 21201 SECURITY: The contractor must have photo ID available when performing work at a VA Medical Center and be willing to present it to VA staff. The vendor is responsible for personal protective equipment. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: KARL STORZ is the only vendor capable of maintaining and supporting this specialized system. Description of market research conducted and results or statement why it was not conducted: In this acquisition, the package requires that the OEM only maintain the equipment.
