The purpose of this amendment is to provide the sign-in sheet and post the answers to the questions received as a result of the Mandatory Site Visit that was held on 04/03/2024 at 2:30 PM. Can the VA please provide reports of the existing tanks? The last 3 reports are attached to this amendment. Can the VA please share a contact of who currently services the tanks? The requiring service does not currently have anyone who services the tanks. Is there any indication of the typical daily use of oxygen? If we are to adequately supply the space with temporary tanks during refurbishment, this information will be critical. This information is currently not available, when/if it is available, it will be forwarded. The RFQ mentions a call back requirement. Please clarify the terms of this service call agreement, namely the duration of the agreement. Once all repairs are complete and the tanks have been recertified the call back requirement will also be complete. The RFQ mentions that The Contractor personnel shall have all the required training and certifications to perform duties as outlined in this contract All certifications and training requirements shall be provided to VAMC personnel. Please clarify which certifications are necessary for this project, if any. Regulations require in order to work on bulk 02 tanks, the contractor is required to carry a ASSE 6010 Medical Gas Installer and a ASSE 6015 Bulk Medical gas/cryogenic fluid central supply systems installers. These certifications shall be included when submitting quotes along with all evaluation factors as outlined in the 52.212-2 EVALUATION-COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES. The solicitation notice mentions procurement of fire extinguisher and suppression maintenance. Please confirm this is incorrect. Yes, this is incorrect. The solicitation notice mentions Price/Cost schedule, will this be provided? The Price/Cost schedule is attached to the solicitation and shall be completed by the contractor outlining their quote for services requested. The solicitation notice mentions Instructions to Offerors, will this be provided? The Instructions to Offerors is outlined in the solicitation under Section E- Solicitation Procedures. If no to above, will formal pricing on company letterhead suffice? Please provide quotes with pricing utilizing Section B.2 Price/Cost schedule. All quotes are still required to be submitted by 04/08/2024 10:00 AM ET.

Background
The North Florida/South Georgia Veterans Health System is seeking to procure Fire Extinguisher and Suppression Maintenance. This will be an annual full-service contract for a base year plus four option years.

Work Details
The objective of the contract is to repair the medical-grade liquid bulk oxygen tank system located at Lake City VAMC. The contractor is required to conduct preventive maintenance, inspection, certification, and related services as described in the statement of work.
The work shall be performed during normal business hours, with some tasks possibly requiring work outside these hours. The contractor must comply with National Fire Protection Association requirements and provide evidence of technical qualifications and previous experience in installing laboratory and healthcare equipment on three similar projects.

Period of Performance
The period of performance for this service is four months from the date of award.
