Amendment 05 (November 15, 2024): The purpose of this Amendment is to: 1) Revise Section 1.6.3, Data Strategy; 2) update the Volume 1: Technical & Management template. Updates to the Technical & Management template largely capture additional information on data and evaluation methods; 3) update the Task Description Document (TDD) template; 4) Provide the model OT agreements per Section 2.0 (d). These models (one AHA and one PHA model) are provided within the Attachment 2, OT Bundle; 4) make administrative updates to Section 1.7, Program Metrics & Milestones, Table 2, Metrics by Health Outcome; and 5) update Section 1.9.2, Program deliverables to account for changes made to Section 1.6.3 and the Technical & Management template. No other changes to the Attachment 2, OT Bundle templates have been made. However, for ease, all Attachment 2, OT Bundle templates are being provided with Amendment 5 and previous versions of Attachment 2, OT Bundle templates have been removed to avoid confusion.

Amendment 04 (October 24, 2024): The purpose of this Amendment is to: 1) revise the proposal due date; and 2) to revise the Volume 3 Admin & National Policy Requirements template provided within the Attachment 2, OT Bundle. All changes are highlighted in yellow. No other changes to the Attachment 2, OT Bundle templates have been made. However, for ease, all Attachment 2, OT Bundle templates are being provided with Amendment 4 and previous versions of Attachment 2, OT Bundle templates have been removed to avoid confusion.

Amendment 03 (September 18, 2024): The purpose of this Amendment is to revise Section 1.9 (b), General Requirements, and to make clarifying edits to Section 2.0 (d), Program Selection Award Information, and Section 5.1 (a), Evaluation Criteria for Award. This Amendment also revises the Attachment 2, OT Bundle specifically the Volume 1, Technical and Management template, the Volume 2, Price Template, and the Attachment 2, Volume 2 Price Summary, Research, Proposer financed spreadsheet template. All changes are highlighted in yellow within the solicitation and attachment templates. Updated versions of the Volume 2, Price Template and the Price Summary, Research Proposer financed spreadsheet template are provided. Changes made within these documents are to clarify intent and streamline the level of detail requested. No other changes to the Attachment 2, OT Bundle have been made. However, a revised Attachment 2, OT Bundle is provided. Proposers should use the Attachment 2, OT Bundle provided within Amendment 3.

Amendment 02 (June 14, 2024): The purpose of this Amendment is to: 1) Provide clarifying details to Section 1.9.2, Program Deliverables; 2) Provide clarifying details to Sections 4.2.1 and 4.3.1 (b); and 3) Update the Solution Summary template (Attachment 1). All changes are highlighted in yellow within the solicitation and the Solution Summary Template.

No changes have been made to the OT Bundle (Attachment 2) in Amendment 02. For ease, the current versions of the templates included within the OT Bundle (Attachment 2) are provided via Amendment 02.

Amendment 01 (May 24, 2024): The purpose of Amendment 1 is as follows: 1) update Section 7.0 of the solicitation to state the Q&A will post only to the HEROES Team webpage; 2) clarify when the model OT will be provided in Section 2 (d) of the solicitation; 3) update the Solution Summary template (Attachment 1) Section 3 (d); 4) update Attachment 2, Volume 2, Price to remove reference to fully burdened in Section III, Price Details (Proposer financed component); 5) provide an updated Attachment 2, Volume 2 Price Summary, Research, Proposer financed spreadsheet. References to fully burdened have been removed throughout the spreadsheet; and 6) provide an updated Attachment 2, Volume 2 Prime Summary, Fixed Component spreadsheet to add clarification regarding fully burdened rates found in note 6A of the template. All changes are highlighted in yellow.

Unless modified within Amendment 01, all remaining attachments within the OT bundle (Attachment 2) remain unchanged.

Under the HEealth care Rewards to Achieve Improved OutcomES (HEROES) program, public health entities and collaborators will have the opportunity to improve the health status of their communities for specific patient populations through the use of a payment model that incentivizes community-based interventions to improve health outcomes across a fixed geographic area. These solutions will investigate a new, regionally focused, outcomes-based financing approach for the healthcare industry, which rewards only positive health outcomes and reduces the healthcare burden on patients, providers, and the economy. The HEROES solicitation highlights four specific health outcomes of interest to the program. These outcome categories were selected because of their significant morbidity and mortality rates despite the availability of effective, evidence-based interventions to prevent such circumstances or reduce their severity. The four outcome categories are as follows:

  1. Maternal Health: Reduction in rate of intrapartum and postpartum severe obstetric complications per 10,000 deliveries.
  2. Heart Attack and Stroke Risk: Reduction in the aggregate 10-year risk of heart attack and stroke for people aged 40-70 years.
  3. Opioid Overdose: Reduction in the rate of emergency medical services (EMS) patient encounters for fatal and non-fatal opioid overdoses per 100,000 population.
  4. Alcohol-Related Health Harms: Reduction in the rate of EMS patient encounters for alcohol-related emergencies per 100,000 population.

Proposers will select one of the four health outcome categories in geographic areas ranging from 500,000 to 5 million people in the general population.

Solution Summaries MUST be submitted in advance of a full proposal, and Proposers interested in proposing to more than one health outcome category MUST submit a separate Solution Summary and proposal for each outcome category/ geographic area pairing.

Background
