The VA Portland Health Care System Department (VAPORHCS) requires Contractors to provide services as part of its community-based Health Care for Homeless Veterans (HCHV) program. The HCHV Medical Respite program serves medically vulnerable Veterans experiencing homelessness who are in need of care above and beyond traditional transitional housing programs offered through HCHV and other services throughout the Portland metro area. The HCHV Medical Respite program supports Veterans discharging from the VA Portland Medical Center or other institutions with medical care needs, providing step-down care in an effort to reduce the need for continued hospitalization or use of emergency services.

The VA Portland Health Care System Department (VAPORHCS) is seeking contractors to provide services as part of its community-based Health Care for Homeless Veterans (HCHV) program.
