Description

Contractor will provide residential care and treatment. To eligible Veterans who need temporary immediate housing placement.

Background
The Department of Veterans Affairs (VA) is seeking a contractor to provide residential care and treatment to eligible Veterans who need temporary immediate housing placement. The contract is part of the Health Care for Homeless Veterans (HCHV) Low Demand Safe Haven program, which is an essential part of VA, providing a gateway to VA and community-based supportive services for eligible homeless Veterans. The central goal of HCHV programs is to reduce homelessness among Veterans by conducting street outreach, providing care, treatment, and rehabilitative services, including case management and therapeutic interim housing assistance by contracting with community providers.

Work Details
The contractor is required to provide short-term 24/7 residential services to eligible Veterans who need immediate housing placement as they seek permanent housing and/or additional care and services. The program shall serve hard-to-reach, hard-to-engage chronically homeless Veterans with severe mental illness and/or substance use disorders and/or other comorbid conditions. The program shall be located within the City and County of San Francisco, CA with easy access by public transportation to San Francisco VA Downtown Clinic and/or San Francisco VA Medical Center.
The contract period includes one 12-month Base Year and four additional 12-month Ordering Periods. The contractor must provide a Quality Control Business Plan that supports the program objectives and associated tasks, as well as a detailed staffing and service plan.

Period of Performance
The period of performance for the contract spans from September 30, 2023, to March 29, 2026.

Place of Performance
The residential programming shall be located within the City and County of San Francisco, CA with easy access by public transportation to San Francisco VA Downtown Clinic and/or San Francisco VA Medical Center.
