Background
The Department of Veterans Affairs (VA) is issuing this solicitation to provide Health Care for Homeless Veterans (HCHV) services in Guam. The goal of the HCHV program is to remove homeless Veterans from unsuitable living conditions and place them in community-based residential environments that offer supportive services to improve their housing situation.

The VA Pacific Islands Health Care System (VAPIHCS) seeks contractors capable of providing emergency housing and related services for eligible homeless Veterans enrolled in their system.

Work Details
The contractor shall provide HCHV Emergency Housing in Guam for eligible homeless Veterans enrolled at the VA Pacific Island Health Care System. Services include:
1. Provision of transitional housing, meals, and case management services for homeless Veterans, with a capacity of 5 to 10 beds depending on site characteristics.
2. Targeting homeless Veterans eligible for VA healthcare, including those transitioning from chronic homelessness or institutions.
3. Immediate bed placement upon request, without denial based on sobriety or legal history.
4. Safe, clean, and secure housing with supportive services, preferably wheelchair accessible.
5. Length of stay limited to 90 days or less, extendable based on clinical need.
6. Engagement with case managers and programming to facilitate permanent housing placement upon exit.
7. 24/7 availability of services with at least one staff member on-site at all times.
8. Provision of three nutritious meals daily and laundry facilities at no cost to residents.
9. Transportation assistance for medical appointments and community access.
10. Maintenance of individual case records for each Veteran served, ensuring confidentiality and compliance with relevant regulations.

Period of Performance
The contract period spans from August 2, 2026, to August 1, 2031, divided into five ordering periods:
- Ordering Period One: August 2, 2026 - August 1, 2027
- Ordering Period Two: August 2, 2027 - August 1, 2028
- Ordering Period Three: August 2, 2028 - August 1, 2029
- Ordering Period Four: August 2, 2029 - August 1, 2030
- Ordering Period Five: August 2, 2030 - August 1, 2031

Place of Performance
The contract will be performed in Guam.
