HCHV Emergency Services | Guam

Background
The U.S. Department of Veterans Affairs (VA) is issuing this contract to provide emergency residential and case management services through the Health Care for Homeless Veterans (HCHV) program.
The goal of the contract is to serve homeless Veterans in Guam who require immediate placement in a safe environment with onsite support services.

Work Details
The Contractor is responsible for providing emergency housing services, including:
- Implementing a Quality Assurance Surveillance Plan (QASP) to monitor performance indicators and standards.
- Conducting general monitoring through announced or unannounced onsite visits by Emergency Housing Liaisons.
- Serving VA eligible Veterans who are homeless or at imminent risk of homelessness, ensuring they are independent in Activities of Daily Living unless otherwise determined by the VA Liaison.
- Providing care, treatment, and rehabilitative services as outlined in the Statement of Work.
- Maintaining housing that is accessible, safe, and compliant with local health and safety codes.
- Offering 24/7 service availability, with a maximum stay of 90 days unless extended by the VA Liaison.
- Ensuring that each Veteran has an assigned primary case manager with a 1:15 ratio or fewer cases.
- Providing necessary hygiene products and maintaining clean living conditions free from pests.

Place of Performance
The services will be performed at the Contractor's VA approved location in Guam.
