Background
The Department of Veterans Affairs has set a national priority to end homelessness among veterans. The Health Care for Homeless Veterans (HCHV) Program is a critical part of the VA, providing a gateway to VA and community-based supportive services for eligible homeless veterans. The central goal of HCHV programs is to reduce homelessness among veterans by conducting street outreach, providing care, treatment, and rehabilitative services, including case management and therapeutic transitional housing assistance by contracting with community providers.

Work Details
The solicitation seeks offers from contractors who can provide short-term residential care and treatment to eligible veterans who need immediate housing placement as they seek permanent housing and/or additional care and services. The program characteristics include residential programming located within the City and County of Sonoma, CA, covering a total period of five years with one 12-month base year and four additional 12-month ordering periods. The program targets homeless veterans who are eligible for VA health care and are transitioning from literal street homelessness or being discharged from institutions. It provides safe, secure housing as well as supportive services such as assistance in securing permanent housing, vocational assistance, income assistance, financial planning, linkage to health care, moderation of substance use, and social and recreational activities.

The solicitation also outlines house rules, expectations, admission practices, case management requirements, mental health and substance use disorder treatment expectations, medication storage and monitoring protocols, storage of personal belongings procedures, absence and cancellation policies, exits to permanent housing criteria, deliverables such as quality control business plan, staffing and service plan requirements, documentation of supportive services, care plan guidelines, critical incident reports submission process, extension requests protocol, admission and discharge reports procedures, satisfaction surveys process, daily census sign-in sheet requirements.

Period of Performance
The period of performance for the contract is from 02/01/2023 to 01/31/2028 with five ordering periods: Ordering Period One: 02/01/2023 - 01/31/2024; Ordering Period Two: 02/01/2024 - 01/31/2025; Ordering Period Three: 02/01/2025 - 01/31/2026; Ordering Period Four: 02/01/2026 - 01/31/2027; Ordering Period Five: 02/01/2027 - 01/31/2028.

Place of Performance
The contract requires the provision of Community-Based Residential Care and Housing Support on a Per Diem Basis in Santa Rosa, CA for the San Francisco VA Healthcare System.
