H-Wave Blanket Purchase Agreement

Background
The U.S. Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Veterans Integrated Service Network 20 (VISN 20), VA Puget Sound Health Care System (VAPSHCS) administers an ongoing program to furnish H-Wave devices to help treat pain for veterans suffering from opioid addiction.

This device will provide the best chance for patients to be more active and engage in rehabilitation to regain control over their symptoms. For veterans suffering from chronic pain, H-Wave home devices offer powerful, long-lasting pain relief to target even the most difficult symptoms and decrease the need for medication.

VAPSHCS intends to competitively establish multiple-award Blanket Purchase Agreements (BPAs) with qualified suppliers as a simplified method for filling anticipated repetitive needs for supplies at fair and reasonable prices.

Work Details
The contractor shall supply H-Wave H4 41000 migraine treatment devices to the VA Puget Sound Health Care System. The estimated requirement is approximately 390 units per year, with a total of 1,950 units over the contract period. The supplier must maintain a 95% availability of the H-Wave devices covered by the BPA.

Shipping is F.O.B. Destination, and all items ordered under the contract must be delivered no later than two business days after receipt of order during standard operating hours. The supplier's express warranty must protect against any manufacturing defects for a period of one year or more.

Period of Performance
The Blanket Purchase Agreement will be effective from June 13, 2025, to June 12, 2030.

Place of Performance
The products will be delivered to various locations including VA Puget Sound Health Care System American Lake Campus in Tacoma, WA, and other facilities within VISN 20.
