Background
The Wyoming Army National Guard (WYARNG) is seeking Non-Clinical Case Management Services to effectively prepare, track, and assess the medical readiness of its service members.

The goal of this contract is to achieve the highest levels of medical readiness and deployability for assigned federal and state missions. This involves creating a coordinated interdisciplinary plan of care for Soldiers from injury, illness, or disease until they return to duty or are medically separated from the organization.

Work Details
The contractor will be responsible for providing comprehensive non-clinical case management services which include:
- Preparing and tracking the medical readiness of WYARNG service members according to their specific medical readiness requirements.
- Assessing the medical readiness status and ensuring that service members meet deployment standards.
- Developing a coordinated interdisciplinary plan of care that addresses the needs of Soldiers from the point of injury/illness/disease through their recovery process.
- Facilitating communication between Soldiers, healthcare providers, and military leadership to ensure effective case management.
- Documenting all case management activities and maintaining accurate records in compliance with military regulations.

Place of Performance
Services will be performed at locations as designated by the Wyoming Army National Guard.
