VACCHCS Emergency Housing (HCHV) - Solicitation   2

Background
The Department of Veteran Affairs Central California Health Care System (VACCHCS) requires contractors to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet their needs and ultimately facilitate the improvement of their housing situation.
Through the HCHV program, VA provides case management services to Veterans and facilitates their access to a broad range of medical, mental health, and rehabilitative services.

Work Details
The contractor will be responsible for providing emergency residential services for homeless veterans in Fresno County. This includes outreach, basic services such as room and board, laundry facilities, therapeutic and rehabilitative services, case management, permanent housing search assistance, financial planning, recovery support, discharge planning, medication management, and extensions. The contractor is required to provide these services in accordance with the Performance Work Statement (PWS) contained in the solicitation document.

Period of Performance
The IDIQ Period of Performance is from 04/01/2023 to 03/31/2028. There are five ordering periods within this timeframe: Ordering Period One: 04/01/2023 - 03/31/2024; Ordering Period Two: 04/01/2024 - 03/31/2025; Ordering Period Three: 04/01/2025 - 03/31/2026; Ordering Period Four: 04/01/2026 - 03/31/2027; Ordering Period Five: 04/01/2027 - 03/31/2028.

Place of Performance
The contract will be performed in Fresno County.
