RFQ for *NEW* 'Base + 4' HCHV CRS Transitional Housing

Background
The Department of Veterans Affairs (VA) is issuing this solicitation for the Health Care for Homeless Veterans (HCHV) program, specifically for the provision of transitional housing services at the Lexington, KY VA Medical Center (VAMC).

The goal of this contract is to provide comprehensive support and services to homeless veterans, facilitating their transition to permanent housing and improving their overall health and well-being.

Work Details
The contractor will be responsible for developing a Comprehensive Individual Service Plan (ISP) within 72 hours of a veteran's admission, which will include permanent housing goals.

Services provided must include daily structured group activities and individual instruction, documented in a Daily Activity Record. The contractor must assist veterans in their housing search, maintain daily progress reports, and conduct bi-weekly collaborative meetings with VA staff to review each veteran's progress.

Additional responsibilities include providing support for personal hygiene, promoting an alcohol/drug-free lifestyle, facilitating community interaction, conducting orientation for new residents, developing written house rules in English and Spanish, implementing a pest management plan, enrolling staff in Homeless Management Information System (HMIS) training, ensuring compliance with security protocols regarding monitoring cameras, and managing drug testing protocols. The contractor must also ensure that all personnel meet federal and state guidelines for residential programming.

The services will be performed at the Lexington VA Medical Center in Kentucky.
