The Department of Veterans Affairs, Network Contracting Office (NCO) 21, has a requirement for HUD-VASH Housing Placement and Case management services for San Francisco VA Health Care System. The solicitation is set aside to small businesses. The North American Industry Classification Systems (NAICS) is 624229, $19M. Contractors must be registered in the System for Award Management (SAM) at SAM.gov to be eligible for award. Please carefully read FAR 52.212-1 Instructions to Offerors and VAAR 852.273-73 Evaluation Health Care Resources in order to respond to this RFQ. Failure to do so may eliminate your response from being considered for award. IAW FAR 16.504, the Government intends to award a Single Award Indefinite Delivery Indefinite Quantity contract as a result of this solicitation. Important: Vendors are responsible for checking SAM.gov for any amendments and must sign and acknowledge all amendments to the solicitation.

Background
The U.S. Department of Veterans Affairs (VA) is seeking a Contractor to assist with the provision of housing placement and case management services through the HUD-VA Supportive Housing (VASH) program. This program aims to place and maintain homeless Veterans in permanent, community-based housing using HUD Section 8 Housing Choice Vouchers paired with ongoing VA case management services. The target population includes Veterans with serious mental illnesses, substance use disorders, and other medical conditions. The contract is specifically for the San Francisco VA Health Care System.

Work Details
The Contractor shall provide housing placement services and ongoing case management to HUD-VASH eligible Veterans and their families in San Francisco County. Key tasks include:

1. Providing case management support using a team approach involving mental health professionals, social workers, nurses, and housing specialists.

2. Complying with HUD-VASH policies regarding documentation and evaluation standards.

3. Conducting home visits and accompanying Veterans to appointments as needed.

4. Supporting Veterans in maintaining housing stability and achieving independent living skills.

5. Participating in weekly case conferences with VA liaisons to discuss updates on Veteran care plans.

6. Documenting all encounters using the SOAP format within 72 hours of service.

7. Implementing the Housing First approach to prioritize quick access to housing followed by supportive services.

Period of Performance
The contract will be effective from January 1, 2025, through December 31, 2029, covering five ordering periods.
