The Department of Veterans Affairs, Network Contracting Office (NCO) 21, has a requirement for Health Care For Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) for Placer County, California in accordance with the performance work statement contained herein. The solicitation does not have a set aside. The North American Industry Classification Systems (NAICS) is 624221, size standard $13.5M. Contractors must be registered in the System for Award Management (SAM) at www.sam.gov to be eligible for award. Carefully read FAR 52.212-1 Instructions to Offerors and VAAR 852.273-73 Evaluation - Health-Care Resources in order to respond to this RFQ. Failure to do so may eliminate your response from being considered for award. IAW FAR 16.504, the Government intends to award a Single Award Indefinite Delivery Indefinite Quantity (IDIQ) Contract as a result of this solicitation. Background
The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is seeking to address the national priority of ending homelessness among Veterans through the Health Care for Homeless Veterans (HCHV) program. This program provides critical support services to eligible Veterans who are homeless, including outreach, case management, and transitional housing assistance.

The goal of this contract is to provide contracted emergency residential services (CERS) in Placer County, California, facilitating the transition of homeless Veterans into stable housing.

Work Details
The contractor will provide emergency housing and supportive services for homeless Veterans under the HCHV CERS model. Key tasks include:
1. Providing secure and wheelchair-accessible housing with a minimum of five beds for men and women.
2. Offering case management services to assist Veterans in transitioning from homelessness to permanent housing.
3. Ensuring same-day admission processes for eligible Veterans 24/7.
4. Providing three nutritious meals daily and accommodating special dietary needs.
5. Maintaining a minimum occupancy rate of 80% for funded beds through outreach efforts and collaboration with VA.
6. Conducting individual case management meetings at least three times per month focusing on housing stabilization and income improvement.
7. Monitoring exits to independent housing with a target rate of 55% or higher.
8. Reporting critical incidents involving Veterans immediately and maintaining individual case records in compliance with confidentiality regulations.

Period of Performance
The contract will be executed over five ordering periods from September 30, 2025, through September 29, 2030.
