Solicitation Questions and Answers Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) in Yolo County for VA Northern California Health Care System (VANCHCS) Could you clarify if a home health agency or laboratory is eligible to apply for this opportunity? This requirement is for emergency housing and supportive services for homeless Veterans in accordance with the HCHV CERS model requirements outlined within the performance work statement. These services are provided at the company's facility/shelter within Yolo County to Veterans who are homeless or at risk of homelessness. Per the PWS, The HCHV CERS Program exists to provide a means of removing homeless Veterans from the street or habitation unfit for humans and placing them in community-based, residential environments with sufficient supportive services to meet their basic needs. In contrast, home health agencies provide services at the client's home, which typically exclude clients without homes. Also, laboratories are not providers of emergency housing services. Can you clarify whether prior experience specifically in emergency housing is a requirement, or if healthcare experience serving vulnerable populations would be considered Healthcare experience working with vulnerable populations would be considered. Background
The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is seeking to provide Health Care for Homeless Veterans (HCHV) through contracted Emergency Residential Services (CERS) in Yolo County, California.
This initiative aligns with the national priority established by the VA Secretary to end homelessness among Veterans, as outlined in the Federal Strategic Plan to Prevent and End Homelessness.
The HCHV program serves as a critical gateway for eligible homeless Veterans to access supportive services and rehabilitative care.

Work Details
The contractor will provide emergency housing and supportive services for homeless Veterans under the HCHV CERS model. Key tasks include:
1. Providing safe, secure, and wheelchair-accessible housing with 10-12 beds for men and women.
2. Offering case management services that include outreach, care coordination, and therapeutic transitional housing assistance.
3. Ensuring same-day admission processes for eligible Veterans without sobriety requirements unless medically necessary.
4. Delivering three nutritious meals daily and maintaining a clean living environment.
5. Engaging Veterans in case management meetings at least three times per month focusing on housing stabilization and income improvement.
6. Monitoring occupancy rates with a target of 80% or higher.
7. Reporting critical incidents involving residents to the VA Liaison immediately.

The contract will be performed from April 1, 2025, to March 31, 2030, with multiple ordering periods:
